Enrollment Referral Process
Kids and Nurses PPEC Centers enroll children between the ages of birth to 20 years old. Our centers and programs are available Monday-Friday year round (excluding holidays). Transportation is provided to eligible children. Enrollment is available to any child regardless of race, color, religion, sex or national origin. Kids and Nurses PPEC Centers comply with the American Disabilities Act.
Eligibility for enrollment to our center is made through a referral and requires an admission process. Referrals to Kids and Nurses PPEC Centers are made by hospital discharge planners, pediatricians and medical professionals. Referrals are reviewed and processed by a healthcare team member at Kids and Nurses PPEC Centers.
The enrollment/referral process begins with a letter from the child’s physician stating the following:
Child’s Name
Date of Birth
Date of Referral
This is a referral to attend Kids and Nurses PPEC Center Monday – Friday between the hours of 7am and 5:30pm for a period of 180 days.
A written prescription including:
• Patients Diagnosis
• Physicians Signature
• NPI Number
• Requested time frame: Monday- Friday, 7am-5:30pm
• 180 Days
-A nursing assessment will be conducted by Kids and Nurses PPEC Center’s nurse.
-We will require health records and current immunization records (provided by the child’s pediatrician)
-Completed enrollment forms will be given to family to complete.
Insurances Accepted
Florida Medicaid, Children’s Medical Services, Florida Healthy Kids, Amerigroup, Staywell, United Healthcare, Sunshine, Aetna, Medkids, Humana, Miami Children’s Healthcare Plan, and Prestige.
Private pay is also accepted.
Insurance participation may vary, please inquire.
